Job Description:

Adult Social Worker 

We are currently recruiting for six experienced Social Workers to join a fast-paced Adult Social Care service. We are looking for confident, experienced practitioners who can manage complex caseloads, work independently and deliver high-quality, person-centred assessments and reviews.

We have opportunities across three specialist areas:

1. Savings Project Social Workers – 2 Positions

We are looking for two experienced Social Workers to support a savings and transformation project within Adult Social Care.

These roles will suit practitioners who are confident working in a fast-paced environment, managing a high volume of work while maintaining quality and professional standards.

You will be required to:

• Undertake reviews, assessments and reassessments of individuals receiving care and support.
• Identify appropriate care and support solutions while considering opportunities for efficiencies and savings.
• Review existing packages of care and identify whether they remain appropriate and proportionate.
• Work closely with individuals, families, providers and other professionals.
• Complete accurate assessments, reviews and recording within required timescales.
• Make sound professional judgements based on risk, need and available resources.
• Manage a demanding caseload and meet project deadlines.
• Contribute to achieving agreed savings and service improvement objectives.

Experience of working on savings, transformation, efficiency or review projects within Adult Social Care is essential.

2. Learning Disability Social Workers – 2 Positions

We are looking for two experienced Social Workers with a strong Learning Disability background, including experience of the Mental Capacity Act (MCA) and court-related work.

You will be responsible for:

• Undertaking Care Act assessments and reviews for adults with learning disabilities.
• Completing complex assessments and developing appropriate care and support plans.
• Applying the principles of the Mental Capacity Act 2005 in practice.
• Completing mental capacity assessments and best-interest decision-making.
• Managing safeguarding concerns and identifying and managing risk.
• Working with families, advocates, providers and multi-disciplinary professionals.
• Supporting individuals with complex needs to achieve positive outcomes.
• Preparing high-quality reports and documentation for legal and court proceedings where required.
• Working in accordance with relevant legislation, statutory guidance and local policies.

Previous experience working with adults with learning disabilities, MCA work and court/legal proceedings is essential.

3. Older People / Review & Reassessment Social Workers – 2 Positions

We are also recruiting for two experienced Social Workers with a background in Older People's Services and/or review and reassessment work.

You will:

• Undertake Care Act assessments, reviews and reassessments.
• Review existing care and support packages to ensure they remain appropriate to the individual's needs.
• Identify changes in circumstances, risks and support requirements.
• Work with individuals, families, carers and other professionals to develop person-centred plans.
• Consider alternatives and community-based support where appropriate.
• Complete safeguarding assessments and risk management where required.
• Liaise with care providers, health professionals and other partner agencies.
• Ensure accurate and timely recording of assessments, reviews and decisions.
• Manage a caseload effectively and work to statutory and service timescales.

Experience within Older People's Adult Social Care and/or significant experience undertaking reviews and reassessments is required.

About You

For all positions, you will need:

• A recognised Social Work qualification and current Social Work England registration.
• Significant experience working within Adult Social Care.
• Strong knowledge of the Care Act 2014 and relevant adult social care legislation.
• Experience undertaking assessments, reviews and care planning.
• The ability to manage a caseload independently in a fast-paced environment.
• Excellent assessment, analytical and recording skills.
• Strong communication and partnership-working skills.
• The ability to make sound professional decisions and manage risk effectively.
• A commitment to person-centred practice and achieving positive outcomes for adults.
